The following documents are required in order for your child to be placed on a preschool roster for the 2024/2025 school year:
- Child’s Birth Certificate
- Child’s Social Security Card
- Child’s Immunization Record
- Custody Papers (if applicable)
- Proof of Residency – bill with your name and current address
- Photo ID – Drivers License
- Proof of Income of anyone working in the household (2 paystubs or a recent W-2)
Please submit these forms to the Elementary School Office by April 21, 2024.
